Wordpress blog has been hacked and unwanted SEO links were added! How do I prevent this?

I woke up this morning to find unwanted links in my blog contend, with added texts so it would look relevant :S.

This is horrible! How do I prevent this from happening again?

Does that mean the whole server has been hacked?
