WordPress Plugin that enable me to show information through a form

I'm looking for a WordPress plugin that let me save information from the BackOffice and identify this information with registration number, and from the FrontOffice visitors can display there informations using there registration number (an InputText where they can write the registration number). Each visitor can see only his related information.
