My WordPress plugin is automatically deactivate due to not valid header (after few days)

I have not any problem in activate plugin , i can easily install plugin and activate it but after two days plugin is auto deactivate error says "your plugin does not have valid header" .

I have already fixed header multiple time , currently my header is following

it is automatically deactivate process after 2 days .mt WordPress version is 4.3.4 running Avada Child theme.
