Wordpress Ninja popups + Mailchimp additional fields

I`m using ninja popups for mailchimp subscription. By default when I try to create a custom template for my popup it seems like within ninja popups there is only option to add EMAIL and NAME fields.

Is there a way to add more fields like Last Name, Address, etc to collect them in mailchimp ?
