Suggest Wordpress plugin / php coding

I want to create system like complaint form using word press for our client property defect.

For example :

Client Login page -- > Complaint registration form open, in that page, all his name and unit number, mail everything automatically display from database + from client side, he need to only type : complaint in text box and then attachment (picture of defected area) and he need to tell his available date and time (appointment)

what kind of plugin can do this ? please suggest me.

from back-end, our salesteam want some website for enter all our client information in our database.
